Description:

An iridium-192 source used for testing was mishandled, exposing six workers, one significantly. The injured worker had placed the source in a jacket pocket, producing a 100 rad whole body dose and a 4,000-6,000 rad dose to the pelvic and thigh regions with effects including skin injury and hypospermia to aspermia.

Consequences: 1 injury (~100 rad).
