Type of event: radiotherapy accident

Description:

Two employees were injured by radiation from a cobalt-60 source in a teletherapy unit. A failure of unit mechanisms caused high doses to the fingers of a technician and a physician, producing radiation burns on the fingers.

Consequences: 2 injuries.
