Description:

During operations with an iridium-192 radiography source an equipment failure occurred. Efforts to regain control of the source resulted in localized radiation injury to the right hand of one individual (whole body dose about 0.5 rad), requiring plastic surgery two years later.

Consequences: 1 injury.
