Description:

During operations with an iridium-192 radiography source an equipment failure occurred, accompanied by inadequate monitoring. One individual received localized radiation injury to the left hand (whole body dose about 0.5 rad), requiring plastic surgery two years later.

Consequences: 1 injury.
