Type of event: radiography accident

Description:

An operator attempted to repair industrial radiography equipment with an iridum-192 source, resulting in a whole-body dose of 77 rad and localized doses of 1,170 rad to the hands and 64 rad to the gonads. The individual suffered radiation injury to the hands.

Consequences: 1 injury.
