Description:
An industrial defectoscope with an iridium-192 source suffered an equipment failure, after which the source was mishandled. One individual received radiation burns on the fingers of the left hand from a localized dose of 2,000 to 3,000 rad (whole-body dose 4.6 rad). A portion of one finger had to be surgically removed.
Consequences: 1 injury.
