Description:

A crew was using a 3,300 TBq iridium-192 source for radiography when the holder cable broke, causing three workers to be exposed to the source. All three received localized doses to the hands resulting in radiodermatitis. Whole body doses were 23 rad, 23 rad, and 20 rad.

Consequences: 3 injuries.
