Description:

A radiographer was injured by exposure to a 56-curie Ir-192 source in radiography equipment. The worker was not using survey or personal dosimeters and failed to manually retract the source to a shielded position. The worker sustained severe radiation burns on the fingers from 5,000 rad localized dose and a whole body dose of 20 rem.

Consequences: 1 injury
